Please complete as much of this application as you feel comfortable with.  You can choose later how much if any of your information gets displayed in the member only directory.  Please remember one of the benefits of membership is networking, so being listed in the directory makes you a resource and mentor and provides you with the same.

        Payroll deduction is $10 bi-weekly. ($260 per year)

        Select this option if you have the ability to use payroll deduction for payment of your membership dues and would like to use this as your payment option. Please download your corresponding deduction form, fill out, and upload on the second page of this form. 

       (Private Practice veterinarians interested in learning more about federal service)

       (Non- veterinarians: Animal Health Technicians, data analysts, molecular and wildlife biologists, laboratory technicians, etc.)

       (One-time Lifetime fee) Any College of Veterinary Medicine can join allowing all interested faculty and students to join for free.

       (No Fee- US Public Health Service Veterinarian)

       (No Fee)

       (Exclusive for Tuskegee Univ. Staff & Faculty)

       (Exclusive for Tuskegee University. Students)

       (No Fee- Student in Malik, Gipson, or Wilson Program)

       (No Fee-  Retired active or affiliate member in good standing at time of retirement)
